2009 Science Congress Forms

The following forms are available as Adobe® Acrobat® format files (.pdf). If you do not have Acrobat Reader version 5, 6 or 7, download a current version from the Adobe Website. You will need to download and print the documents listed below.

Science Congress Rules Brochure : Describes what students and mentors need to know about participating in the regional Science Congress. We are affiliated with the International Science and Engineering Fair (ISEF), so we follow the international fair rules. The ISEF forms are our official forms. Be aware that your project may be rejected if safety and ethics protocols are not followed. Please read the brochure carefully!

International Rules and Guidelines booklet: All high school researchers and their mentors must become familiar with the contents. Mentors of younger students are encouraged to read this booklet.

The Student Handbook is a four page summary about doing a succesful project. You may access these items online at www.societyforscience.org/isef/document/index.asp.

Student Checklist : This will ensure you don't forget anything.

Junior High Entry Form : A required form for students in grade 6-8. The form gives us basic information about you.

Senior High Entry Form : A required form for students in grade 9-12. The form gives us basic information about you.

Form 1 : Sponsor Checklist & Safety Assessment. A required form for grades 6-12. Reveals whether your project obtained appropriate approval. Must be signed by your sponsor with a date prior to experimentation.

Form 1A : Student Checklist & Research Plan. A required form for grades 6-12. 1A describes where and when you did your research. The attachment outlines your plan. Do not confuse the plan with an abstract.

Form 1B : Approval Form. A required form for grades 6-12. Student and associated adults acknowledge their responsibilities. Dr. Pavelka's signature shows you received approval for a project that required prior approval.

Abstract: Required for grades 6-12. For the regional Science Congress there is no special form. Type it on a single sheet of paper and write less than 250 words.

Certain projects may need additional forms to comply with ISEF rules. Before you begin your project, know if this applies to you! Helpful information about following protocol and determining what forms are necessary is found in the ISEF Rules Wizard.
