Remember: Don't just fill out a request form and send it in - it will get rejected. Your Chapter Leader must sign your request form to authorize your use of the library.
 | Who Can Borrow Tapes? |
A Chapter Leader may authorize any dues-paying LEAH member of their chapter to borrow tapes from the video lending library. However, the chapter leader must accept responsibility for any lost or un-returned videos. This responsibility includes contacting those people who have borrowed videos and failed to return them, and payment to replace videos that are never returned. You as a Chapter Leader are responsible to make the list of available videos accessible to those that want to use the library.
| How much does it cost? How long may we keep them? |
The cost for each rental tape is $4.00. If ordered for a specific play date, your videos will be mailed to you approximately two weeks prior, to ensure you have them well in advance. Please return the tapes promptly after your group showing. Keep in mind, we have only one copy of most tapes, and one rental can put the tape out-of-circulation for approximately one month. The individual requesting the tape is the only person authorized to use the tape. Please do not rent a tape and pass it to other individuals. You are expected to return the tape as soon as
you have finished viewing it. We fully expect the tape to be back to the library no later than one month from the date of the first shipping. You may request no more than 4 videos in one order. Please be considerate of others and return tapes promptly.

Chapter Leaders or your chapter's Video Coordinator can authorize you to borrow a tape from the library. You should give an alternative choice, just in case your first choice is not available. Include your payment with the order: Checks should be made out to NYS LEAH.) Remember; The Chapter Leader must sign the form to authorize use of the library.
| Where does the S4.00 fee go? |
The $4.00 charge covers such things as postage, labels, mailers, and other small expenses. At the end of the year, any moneys generated over and above these expenses are used to purchase new tapes.
